Dear Reader Being a Functional Medical doctor, I am always getting patients who have tried just about everything before they can get some professional advice. So a tall challenge is when patients come to me telling me that they do not want any medications but want to get their pain, cholesterol, weight, and sugars under control. Can You Live So Healthy You Never Need Meds? We’ve all heard it, or maybe even believed it: If I eat clean, exercise daily, avoid toxins, and meditate, I won’t need medications. Over a decade ago, I entered Functional Medicine thinking lifestyle was the answer to everything. And don’t get me wrong, it’s foundational. Many of our patients just need to tweak their lifestyle, and they find true lasting health. But with years of experience and countless patient journeys, I’ve come to realize: sometimes lifestyle alone isn’t enough. Let me share a story. It’s a blend of many patients I’ve seen (with all identifying details changed for privacy), but the theme is real and familiar. Meet Sima. Sima came to me after an incredible self-led health transformation. She had lost 35 pounds, was lifting weights daily, and was dedicated to a plant-based, whole food diet. Her cholesterol and triglycerides had improved, but still weren't ideal. Her A1C (a marker of blood sugar control) was borderline. Her insulin levels were too high. And her coronary calcium score showed early plaque development, despite her "clean" lifestyle. She was perimenopause, and things were about to get worse if we did not find a sustainable solution. She was doing everything right. But her numbers told us there was still metabolic and cardiovascular risk lurking in the background. Add to that: her father had died young from a cardiac event. She came to me because she wanted to defy her DNA with her lifestyle. Over the next two years, we worked together closely, adjusting her nutrition, training, eating schedule, and supplements. Slowly, we made progress. Sima was highly motivated. She was even willing to try animal-based protein because it was suggested she needed it. But it backfired as her cholesterol went up. So, back to the plant-based eating. But even with all her effort, her cholesterol levels plateaued, and she couldn't drop past a certain weight. Eventually, we started a very low dose of medications to lower cholesterol. I had to send her to an integrative cardiologist who supported this move. That helped, but still didn’t move the needle enough on her weight or blood sugar. After much conversation, reflection, and resistance, she agreed to start a GLP-1 medication. These are the weight loss peptides everyone from Hollywood to Bollywood is using to reach unbelievable weight loss and youthful looks. Six months later, her A1C dropped to 4.9. Her insulin was at 3.5. Her weight came down to a healthy, sustainable 145 lbs. Her food cravings disappeared. She had more energy, better focus, and finally, peace with food. She was able to exhale! Here’s the point: Sima didn’t fail. We often expect lifestyle to do all the heavy lifting, but it’s just one part of a bigger picture. As physicians trained in Functional Medicine, we walk a fine line between honoring lifestyle and knowing when to add in tools like medication. And we do it without judgment, without shortcuts, and always to support healing, never just suppressing symptoms. I understand the internet makes it seem simple: “Just eat this way, exercise, and everything will reverse.” That advice may work for some, but many people are carrying complex, layered health stories that can’t be untangled by diet alone. So no, medicine isn’t about absolutes. It’s about nuance, context, compassion, and customization. That’s what we do here. We start with lifestyle, always. But we don’t stop there if your body needs more support. The notion that every doctor is in bed with pharmaceuticals is laughable. I have not met a pharmaceutical rep in 10 years. Before that, I used to take samples from them to help a patient or two get their medications for free. If you have ever worked with a doctor enough, you will see they try to help. Many stay awake at night wondering what else they can do, have they missed anything, and did they do their best? This is an occupational hazard. The internet may be loud, but your health doesn’t need hype. Health isn’t built on headlines. It’s earned, day by day, with the right guidance, the right tools, and the right care. I'm honored to be on this journey with you. Live healthily and iconically, Dr. Nisha Chellam

Dear Reader, Your Heart Doesn't Send a Calendar Invite Before It Attacks Here's the uncomfortable truth about heart disease: it's a quiet tenant. It moves in, makes itself at home, and gives you zero warning, until it does. In roughly one-third to one-half of fatal cases, sudden cardiac death is the first and only symptom. No chest pain. No dramatic movie moment, clutching the chest. Just gone. The good news? Most of it is preventable. But only if you know what to look for. The 5 Risk Factors...
